Dual Arachnid build, couple issues

Building a dual reverb using the arachnid & EEPROM builder. Had the FV-1 soldered by pedalpcb. Finally got everything wired up last night to test and I have a couple issues.

1) loud buzzing when bypassed
2) side A (shown on right when pots are face up) - when engaged the volume drops to about 10% (volume knob is cranked, mix at noon)
3) side B (on left when pots face up) - when engaged the volume is at about 5% (volume knob is cranked, mix at noon)
4) LEDs are slightly on when bypassed. When footswitch is engaged LED lights up as usual but when disengaged, neither LED turns fully off. They both drop down to a dim light. (can be seen in the first picture....kinda. the left side is engaged, the right side is disengaged.)

I'm pretty new to building pedals. Making lots of rookie mistakes. I've only built a handful at this point and so far everything has worked pretty well. Not really sure how to even start trouble shooting. My knowledge here is at a deficit. Any help would be greatly appreciated.

If the LED isn't switching on and off properly there is most likely an offboard wiring issue.
