I suspect a bad or weak connection because this pad isn't filled with solder (red), and an exemple of probable short, caused by some remains of solder, or something (green) :

I haven’t looked at the schematic but those op amp voltages look off. I’ve never seen OPA2134 ICs with that print before. Where did you get them? Could you swap for two TL072s?

Hi,,, The OPA's were given to me in a sleeve from a friend who was going to replace the chips in a mixing board but never got around to it. They're legit.

I used TL072 and got roughly the same voltages.
