    I just checked mine, the voltages are close to what GGG says they should be. I was going to suggest looking at resistors on the feedback path from Q2 emitter to Q1 base. So what resistor was it, I'm curious.

    D3 and D4 are clipping diodes, the anodes are opposite each other, if you used germanium diodes the forward voltage on them is low so they may look like a short, as long as you have them installed correctly according to the board they should be good.

    The op amp isn't biased correctly. You should have about 4V on all pins, except pin4 = 0V, and pin8 = 9V. This voltage is set by R101, R102 and C101. It is applied to op amp at pins 3 and 5. Check voltages on R101 both sides of resistor.

    An easy way to figure the caps, but not always 100%, is to de-solder the cap, power up the circuit and measure voltage across the pads of the missing cap. One way the it will be positive and the other negative. When its positive, where the red lead is, that should be the + lead of the cap.
