One of the suggestions from my original post said to pull pin 7 (ground) and that is what actually made it briefly work.

I went back to it today and examined the solder side of the board. I finally found 2 unsoldered pins and a capacitor with bridged leads. I also re-inserted pin 7 in U3. Works very well and one of the most bizarre things I've ever heard. I need to keep in mind to always go back and check the...

After lifting pin 7 the circuit did what it is supposed to. For a while. Now the major signal disruption stopped and I get a little distortion. I’m going to take this over to Troubleshooting as recommended. Lifting pin 7 leaves the chip without a ground if that matters.

My PCB is missing the ground attachment next to the 9V.

I just did a quick lookup of voltage dividers and the voltage at pin 3 makes no sense. It should be between 6 & 7 volts if my arithmetic is correct. Some of the examples I saw show one resistor being tied to ground.
