All the problems you mentioned are due to the foot switches not working correctly. It's possible you overheated them when applying solder. Use a multimeter to verify they are switching correctly.

Make sure the op amps you're using are genuine

Are they meant to be a ground trace in lieu of a plane to prevent parasitic capacitance between the traces either side of them? They all seem to be much thinner than their neighboring traces

It looks like the flush mount is meant to be used with one of the faceplates, and you drill a large circular hole big enough for the power jack to fit thru. The faceplate covers this and the power jack is then flush with the face plate.

I would reuse the pots, LDR's, transformer, any socketed IC's, and the transistors if you can remove them cleanly. Everything else really isn't worth the time.

I would love a scope but I'm intimidated by the up front cost. Is there a "value" one out there that would have the features I'd need for a pedal but is less expensive? It's hard to justify as a "sometimes" tool.
