SOLVED Headphone Amplifier: Only getting mono output

I have a SUPER dope headphone amplifier combo to share with everyone via a build report, but there is one small problem: I'm only getting sound in one headphone (mono) and I don't know why. I feel like a troglodyte. I'll attach photos but not sure if they'll be much help (but you can see how they're wired).

What am I missing guys?
