Power loss when cord plugs in

I’m having a wierd issue... I’m got a dual circuit build, using an order switcher, 3PDT boards, all pedalpcb. Had it all working done yesterday, but the graphics weren’t on so took it all out of the box. Replaces all today. Plug power into dc adapter, all is ok, borg leds light up. As soon as I plug a cable into either or both I or O, I loose power.

Sounds like the tab of the tip on your in/out jack is making contact where it shouldn’t be. Post some pictures, could be wired wrong too.
