Didn't want to start another thread but this design caught my eye so I made a board for it, sort of. The main thing I wanted was the phase inverting switch. So this board only contains the Blend half of the schematic, not the splitter. Also doesn't include the power supply filtering / 1/2 voltage reference section because I ordered it on a separate board to make things most cost effective.

I used a switching jack on the invert-able side of the blend pot to be able to negate the blend pot. Plugging into (OUT2) breaks the pot connection and the ring adds in a 1M resistor to sleeve GND. Thus I can use IN2+(OUT2) and have just an inverting buffer. A little bit confusing but worth it for how I mostly want to use it. Otherwise the pot could be mounted on the board.
Enclosure is a recycled 1590BB, the previous holes are filled with quicksteel putty, board is floating on wires, and there's no LED yet. Not sure if I'm going to add the splitter section but there's room for it if I change my mind later. Works great as is, nice and quiet. That phase inverting switch is really useful for running stuff in stereo.
