Random tremolo

A friend asked me to fix this, a broken output wire was the only issue but I was wondering if anybody knows what it is. I looked through some of the most common diy tremolos like the EA and didn't see anything that looked exactly like this one. There's a 4558, a UA741, and two J176. Sounds good, 4 waveshapes, can get nice and choppy with the bias knob. I'd take picture of the underside but I feel like I'm going to break it. Pretty fragile.
