Solved - I took the pedal out of the enclosure and cleaned with a toothbrush and -OH. I noticed 2 solder blobs touching on the back side. Cleaned that up, reinstalled it and it works now. Big hint - I don't think the LED's should have been that bright in the video (the ones inside for clipping).
