    These look great! You can improve the strength of the octave by using diodes with matched forward voltages. These differ from part to part so use a DMM to find a pair that are similar and chuck them in.

    No problem! I'm not familiar with that jack but as long as they have a stitched tip they'll work! Only the middle two need to be switched, the outer jacks can just be regular old mono jacks.

    Can you post some photos of the board, component side and solder side? Could be a short, incorrect component, faulty part, incorrect wiring etc. There's a lot of options, let's try to narrow it down.

    Try using a DMM to see if the LED is still functional. It might have died for some reason. If it's dead, replace it. If it's working, check you're voltages on TL072 in IC5A (particularly pin 1) and Q1A, these are the components controlling voltage to that LED.
