    If I'm reading the schematic right. Fv-1 pin 6 should be similar to pin 8 about 3.3v.

    The bias trim should adjust the voltage for q2. Are you able to set it to 4.5-5.0v. Have you done a reflow on the solder joints and cleaned the pcb?

    Pictures speak a thousand words. Reflow the solder joints around the dry signal and mix pot. Check component values as well.
