Awful Waffle Issues

So, I finished this pedal, had it working okay, although I thought it was a little hot. It was supposed to be a gift for someone. I thought I would try swapping out the regular LED for one that flickers, to give it a flame effect. I tried it out shortly after that and it seemed to work okay. Upon giving the gift to my friend and plugging it in, it immediately gave this screechy feedback sound and a heavy bone-crushing pop when turned on, even with his amp turned way down. Any ideas? I am thinking the LED has something to do with it, but I am not sure.

Thank you. Is there a simple way to check that? I don't see any visual evidence of that.

Maybe someone here will if you can supply images? What type of power supply was your friend using?
