The PPCB Minnow might also be worth a look. It’s based on a Tom Oberheim designed filter, and it’s got some cool modulation options as well.
The Korg MS-20 filter is another synth classic that seems to pop up in DIY somewhat frequently. Not sure if anyone currently sells a guitar friendly PCB.

Given that's from DEFX, it's debatable whether or not the PCB is "friendly".
Maybe the
DEFX TRESONATOR, based on Frostwave's "Resonator" that's also based on the MS-20 — the Tresonator is much more friendly, it's meant for a wah enclosure, but who says it has to to go in a wah-shell? I have a Tresonator to build up. So much to do, so little time left in life.
Also,
DEFX has now the Q2, a conventional pedal-based PCB of the Frostwave Resonator — whether the PCB is guitar friendly (meant to fit a Hammond 1590D or similar), well once again that's subjective.

BARN OWL, by Peddler Effects. MUCH more sensible-sized — fits a 1590BBS portrait enclosure. I believe
@DGWVI has a build-report of this PCB. I'm aiming to get to this, eventually, hopefully before the next Millennium.
KS-20, by Kassutronics, is meant for Eurorack (or similar), not sure if there's still pre-fabbed DIY-PCBs available for it or not, but everything needed to full-DIY was put up on GITHUB.

DiabloChris6's Madbean thread about his Somnambulist HPF based on the MS-20. Looks like he made his own PCB, and he does have some OSHPARK builds available, but I don't know if the Somnambulist's MS-20 circ is amongst them. [*checks*] NOPE!
