This one is lifted from the D.A.M. site, "Sounds pretty rad on bass!", says the Captain, which is good enough for me - and hopefully you too?
It's from July 1977 and uses 2 opamps, one of them to do the fuzzy thing with GE diodes and the other for a cut/boost bass and treble tone control.
Also pointed out: "it uses two batteries and has a dual-rail power supply."
Could be tried on vero, but I'm lazy and don't like vero too much ... so I'd prefer a PCB, pretty please?
This one from 1967 is quite interesting too, especially when adding an LFO for "rhythmic fuzz".
