I feel like lug 3 of the volume pot should have been connected to lug 1 of the tone pot, instead of lug 2. Like @Bricksnbeatles mentioned, that's just not how the original was wired up, but I think it should have been.

It should be, but signal can't pass through open relay contacts so either it's not in true-bypass mode, or the relay isn't switching correctly. (sticking in the Active position)

Have you tried switching it into true-bypass mode to see if that makes a difference? Hold down the footswitch while applying power, the LED will blink to indicate you have changed modes.

It's not a bad idea for shielding but it's not required. I feel like connecting to a ground pour (versus four small isolated pads) gives a more substantial physical connection, but I don't always do it. (same with 9mm pots)
