That's for the 2n5458 - you have the option of the through-hole one, or a surface mount one. The circled pads would be for the surface mount if you go that route.

Ok, thanks. I have the through hole component. Wasn’t sure if there was a second one needed or something that was surface mount. I see now, it is an option of either/or.
