Breakout board wiring

How would I go about wiring this?

Far left (unlabeled) pad of the breakout board goes to the input jack tip.

Far right (unlabeled) pad of the breakout board goes to the output jack tip.

IN/GND/SW/OUT connect straight across.

BP connects to BUFFER.

Ignore the Mute pad. You can wire it up if you really want to (Connect to bottom center lug of 3PDT) but most folks just ignore it when using a breakout board.

I don't know how he knows all this stuff but has only ever posted one build report. just keep hanging around but I swear he builds nothing.
