I've seen some threads about this but I don't have a clear image in my mind of how this goes. Is my picture below correct (blue is the shield, red is core)? Do I need both ground connections 1&2 or does that create a ground loop? I'd like to start doing this on fuzz builds. Thanks

I used coax to try and quiet a minnow build and that is how I connected them. Yes you want both since one grounds the board and the one from the coax diverts environmental noise
 
That wiring looks to be correct.

A wire from each jack to the PCB isn't necessary, but in this application all grounds do need to be connected.

I like to link the in/out rings to the DC jack center and bring a single wire to the PCB in a daisy chain. Mostly because wire-to-board connections are the bane of my existence.
