I'd love to say that I sorted this one out, but no luck. Chuck made a recent comment about some TI CMOS chips being noisy, and that could possibly be in play here, but I don't have any non-TI options to test with to see if it helps.

Nice work! I have very mixed emotions about this pedal after building it myself. Does yours by chance tick when the Oscillator is turned up? I noted this problem a while back and others have said the same thing (including on the actual EQD pedal), so I'm curious.

Sure, just jumper from the middle hole to one of the outer holes depending upon if you want symmetric or asymmetric clipping. If you don't know which you want, just stick something temporarily in there and try it both ways before you solder it in.

When I made the template the intent was to use the smaller Lumberg jacks. I'm shocked that you are able to squeeze in the standard open ones. I'm glad it has been working out for you!

No luck. I haven't messed with it much though. At this point I pretty much hate this thing and I'm not even sure why I wanted to build it in the first place :) Maybe I'll dust it off and play with it a bit more, but I'm not super motivated since it was a very niche build to begin with.
