Attached is a vero layout for a daughterboard I've used in the past. The pot (100k) is an input trim adjustment and can be internal or external depending on your application.
EDIT: IN is taken from the main instrument circuit path, the TRS connections are for the AUX input, and OUT goes to the input of the headphone amp circuit.
