Recommend Me a Clean PreAmp

I spec'ed relatively large SMDs (1206 resistors, SO-8 opamps) and all the caps are through-hole. There's a new version in the works that runs internally at +/-15V and can directly drive any power amp, but the current version already has plenty of headroom, especially if you run it at 12-18 volts.

If you end up going that route PM me and I'll run you through a few mod options to optimize for your specific use case. The most obvious ones would be input impedance and mid frequency choices, both of which I've messed with a lot over the years. All it takes is a few simple component value changes, super easy stuff since I have the whole thing modeled in LTspice.
