Tried the pedal with a different 9V power adapter and it works. Note to others: doesn’t work with a D’Addario YS12 AC-DC 9volt power adapter I use to test all my other pedal builds. Works with a Boss PSA-120. Marking this solved.

On average, I'm waiting a month for single orders of 10-12 125B enclosures. Been this way for quite a while. The shop does fantastic work, I'll say that much. If I run out, I'm using Stompboxparts or Small Bear in the meantime. Looking forward to paint production upgrades improving wait times...

Success!! I separated the Time out and Trigger/Gate jacks and the Loop Send and Loop Return jacks and placed electrical tape between them. When I reassembled and tested, I have all effects now. You were spot on about the potential of a short in this area. I’ll mark this solved. Thanks!

Maybe try using the Stomp switch breakout board for input and output connections instead of wiring to the switch lugs?
