3pdt Wiring Board w/ LED help

can someone provide a schematic I can follow to hook up this wiring board to my pcb, input and output jacks and dc jack? I will not be using a battery. The available pads on the pcb are: “I”, “+”, “G” and “O”

The problem is I don’t have the breakout board above but do have the 3pdt with led as attached. Still looking for a schematic based on what I have.

oh, oops.Gotcha.

The I/O on the breakout go to the jack tips.
The RCV / SND go to the pcb I/O.

Yep Getting there ..... and pcb + G to breakout board +V G (and then breakout board +V G to + G on dc jack?). You‘ve been very helpful and I really do appreciate the input!!!
