Can someone help me understand the lugs on this input/output jack?

I think I've wired them up wrong by assuming the tip lugs were both the signal, I can't read this datasheet though

Do you have a multimeter? Plug a cable in and do continuity checks

That sheet says 1 = Sleeve, 2= Tip ... should work

This is the only part of the diagram you need. When the plug is inserted, it disconnects 2 and 4.

The in and ou wires are soldered to both 2 and 4, so it should be ok. There just would be no switching.

Could we see your footswitch wiring. In fact, can we see the whole thing ?

Brett has a point, if it's a stereo version, then the ring lugs is already grounded to the sleeve, and connecting the tip to the ring, you would just ground everything.

EDIT : A quick test you could do. Insert your cable in the jack and test with your multimeter for continuity between sleeve and your signal wire (or sleeve and tip at the other end of your cable). There should be no continuity

I’ve connected to the tips in this pic. The other is the sleeve and the back is the ground, natch.
