a.) Take that file and open it in a graphic design software. You'll need to size it to be a match for the 125B. Then just take measurments from the center to all the hole locations.
I'm leaving work too soon to fire up Illustrator for more exact measurements but this should get you there. Done with the measure tool in Adobe Acrobat Pro - not sure if the free version has the tool or not.
I would be very cautious with the PCBGM template...
It looks like it would be very easy for the 1/4" jacks to interfere with the components on the main PCB. The knobs are set way too high in the enclosure for my comfort.
